I, the student, or parent/guardian on behalf of the student, do hereby agree to participate in the activities at Shore Kickboxing located in the Yorktowne Plaza, 2970 Yorktown Boulevard, Unit 4, Brick, NJ 08723 and outdoor surrounding areas. By signing this Waiver, I am stating that I have read this entire Waiver prior to signing. I recognize the risks of injury that are common to any martial arts and fitness program that my child or I participate in. On behalf of myself, members of my family, our heirs, executors, administrators and assigns, hereby forever waive and release, discharge and hold harmless, Shore Kickboxing LLC, representatives and agents from and against any and all claims, actions, causes of action, damages, costs, liabilities, expense of judgments, including attorney’s fees and court costs, that arise out of my participation in this program, including any injury, loss or damage to my person or property howsoever caused, arising out of or in connection with my taking part in fitness and martial arts classes and activities and notwithstanding that the same may have been contributed to or occasioned by the negligence of Shore Kickboxing representatives or agents. I hereby execute this Waiver permitting myself or my minor child to participate in Shore Kickboxing's programs and activities.
